innercare.org is an informational website. It is not an electronic health record (EHR). It is not used to create, store, access, or retain patient medical records. Clinic records are kept in Innercare’s separate clinical systems.
This page is not the Website Privacy Policy (cookies, donations, and similar website information) and is not the staff SMS terms.
Our commitment
Innercare and its employees are committed to protecting the privacy of your health information.
As an Innercare patient, you have the right to:
- Request confidential communication of your health information.
- Request a restriction on certain uses and disclosures of your information.
- Inspect and receive a copy of your health record.
- Request an amendment to your health record and obtain a listing of who has been given your health information.
- Obtain a copy of our Notice of Privacy Practices.
Printable copy and complaints
The printable Notice of Privacy Practices PDF and the HIPAA complaint process remain on our HIPAA page. We have not changed that page or its PDF.
If you believe your privacy rights have been compromised, you may file a complaint with the onsite Clinic Manager or the Compliance Officer by phone or by completing the HIPAA Complaint Form on the HIPAA page. You may also contact:
